18th Annual Free Sports Physical Event on Friday, June 5, 2026 – 4:30pm Check-in

Get Ready for the Season with a Free Sports Physical!

You’re working hard to prepare for the upcoming sports season, but don’t forget—before you can step onto the field, you’ll need a sports physical. The Center for Orthopedics is offering quick, easy, and free physical examinations.

Event Details:

• Date: Friday, June 5, 2026
• Time: 1:00 p.m. – 5:00 p.m.
• Location: 5001 Transportation Drive, Sheffield Village, Ohio 44054

How to Participate:

1. Visit your school website to download the 2026–2027 Ohio High School Athletic Association (OHSAA) Preparticipation Physical Evaluation Consent form.
2. Complete and sign this form with your parent or guardian prior to your exam, or bring your parent/guardian with you. Forms will also be available at the event.
3. Bring any relevant medical records you may have.
