Insurance and Payment

We accept most major insurances. Please remember HMO plans require a referral. Please be aware that your appointment may need to be rescheduled if your primary care physician did not get your referral from your insurance company.

Q. When is payment due?

Payment is expected at the time of treatment.

Q. What happens if I can’t pay my balance in full?

If you can’t pay in full at the time of treatment, we will accept monthly payments based on the amount of your outstanding balance, as follows:

If your balance is: Your minimum monthly payment is:
$50 or less The entire balance due
$51 – $500 $50
$501 – $1,000 $100
$1,001 – $2,500 $200
More than $2,500 10% of the balance due

Q. When are payments due?

Payments are due by your next scheduled appointment or on the 15th of each month.
